In-Person Deposit Form

By filling out this form, I agree to the following: I hereby declare that I am over 18 and am competent to sign this agreement. The tattoo deposit is part of the total price of my tattoo and is non-refundable. If I need to make any changes to the scheduled tattoo appointment date, I must inform CJ Tattoo Studio at least 48 hours prior to my tattoo appointment. If I do not show up for the tattoo appointment, I forfeit my deposit and a new deposit is required to set up a new appointment date. If I fail to show up 2 times without advanced notice, CJ Tattoo will not accept any more booking requests from me. I will arrive on time to my tattoo appointment, CJ Tattoo may not accommodate me if I arrive past one hour of my appointment window. I understand that the artists need to attend to other clients that may be scheduled on the same date. After the artist sends me the design, we can make small changes to the design up to 2 times for free, if I need to continue making adjustment, or change the design theme, another deposit may be required. I understand arrivals over 30 minutes late may be subject to a $50 fee. I understand the deposit is non-refundable, I will not ask for a refund.
